Experiences from implementing a reimbursed medication review service in ambulatory care in Slovenia

In the past, we have seen several developments in the area of Pharmaceutical Care, of which medication review is perhaps one of the most promising.

In 2016 the efforts of many enthusiastic pharmacists in Slovenia resulted in reimbursement for an advanced medication review that was introduced at the ambulatory level. The Slovene Chamber of Pharmacy introduced a certification system in 2014 which officially defined two types of medication review services. Apart from the advanced medication review (Type 3) with a focus on prescribing quality, also a medicines use review (Type 2a) with the focus on adherence issues. Both services were written into the new Pharmacy Act (December 2016) as well as the medication reconciliation services that are planned in hospital settings as part of the seamless care procedures.

In this seminar, participants will reflect the past and current Pharmaceutical Care research. They will discuss the most important developments from their own projects as well as other national or international projects that are worth consideration and implementation in pharmacy practice. Finally, they will reflect on the actual or potential impact pharmacy practice projects had or could have. This will help them prepare a research strategy with the potential to achieve excellence in pharmacy practice.
